Please follow our safety guidelines during your visit:
Arrive at your appointment time to avoid wait room crowding.
Sanitize hands upon entering the building or wash hands in the second floor restroom.
Masks are optional but please wear a mask if you’ve been traveling or at a large gathering.
If you are not feeling well, please text me before your session to determine if we can keep your appointment.
How to Prepare for your Visit
Paperwork
Please print out and complete the following forms prior to your appointment time. If you don’t have access to a printer, let me know prior to your visit so that I can have them ready for you to fill out when you arrive. Give yourself about 15 minutes to complete the forms.
What to Wear
What to Expect- Acupuncture visits
Your first visit will last around 90-120 minutes. I will conduct an in depth interview regarding your current state of health and your health history. Then I will look at your tongue and listen to your pulse.
Combining all of this information will help me to place your concerns in a holistic context, at which point I will determine an appropriate treatment plan for you. This may include acupuncture &/or Chinese herbal medicine and nutraceuticals.
After placing the needles in a prescribed manner, I will allow you to rest quietly for about 30-45 minutes.
What to Expect- Herbal consultations
Your first visit will last about 60 minutes. I will conduct an in depth interview regarding your current state of health and your health history. Then I will look at your tongue and listen to your pulse.
Combining all of this information will help me to place your concerns in a holistic context, at which point I will determine an appropriate treatment plan for you. This may include Chinese herbal formulas in tea, tincture, pill or powder formats &/or supplements.
Forms of Payment & Insurance
Forms of Payment:
I accept cash, checks or cards. Cards are held on file electronically to insure a touchless transaction. A 3% processing fee will be added to all credit card transactions.
I am a preferred provider as a Licensed Acupuncturist with the following insurance companies:
Aetna, Blue Cross Blue Shield, Coordinated Care, First Choice, Humana, Lifewise, Premera, Regence, TriWest, Uniform, United Healthcare, Voucher, and WellCare.
I may submit claims on your behalf as an ‘Out-of-Network’ provider with: Cigna and Kaiser
I am happy to provide you with a ‘super bill’ for reimbursement from your insurance when I am unable to submit a claim.
